The fibrinogen and thrombin components of Beriplast® P Combi-Set can be applied using a wide range of devices. The devices shown here are compatible with a Luer-Lok® fitting and/or can be used with the syringes supplied for the Beriplast® P Combi-Set.

The availability of delivery devices varies from country to country. Please contact your local CSL Behring representative for details of devices available to you.

First, reconstitute the fibrinogen and thrombin components of Beriplast® P Combi-Set following the manufacturer's instructions then apply using one of the following methods:

Separate Application of Fibrinogen and Thrombin Solutions

1. Apply the fibrinogen solution to the tissue site which requires sealing and overlay with the thrombin solution.

2. Hold the tissues requiring sealing in place for a few minutes until a fibrin clot begins to form and adhesion is achieved.

Joint Application of Fibrinogen and Thrombin Solutions

For joint application of the fibrinogen and thrombin solutions, the application kit supplied in the Beriplast® P Combi-Set package can be used:


Pantaject®

1. Insert the blue Y-piece into the white syringe holder. The fixation device should be bent at 90‹ in the middle. Insert the filled application syringes (remove needles) firmly into the Y-piece. Snap both syringes into the syringe holder. Attach the grip plate to the syringe plungers to prevent jamming of the plungers and to ensure smooth forward movement.


2. It is easier to assemble the Pantaject® system if, after the Y-piece is attached to the syringe holder, the device is bent at 90‹ in the middle.

3. The syringes should then be pushed securely into the Y-piece before the syringe holder is snapped into position.

4. Fix a suitable device (eg needle) into the Luer-Lok® fitting. To ensure that the system is free from blockages, hold the Pantaject® so that the needle is pointing upward.

5. Beriplast® P Combi-Set is now ready for use. Apply the fibrin sealant to the site by gently pushing down on the plunger connector. NB: to prevent the needle from becoming blocked, do not attach it to the Pantaject® until immediately before use.

Spray-tip

When the surgical field is extensive, Beriplast® P Combi-Set can be applied using a spray tip.

1. Assemble the Pantaject® and check that the fibrinogen and thrombin solutions have not become mixed at the head of the Y-piece causing a blockage. Keeping the Pantaject® upright, attach the spray-tip to the Luer-Lok® fitting of the Y-piece. Apply Beriplast® P Combi-Set by pushing down slowly and evenly on the plunger connector to produce a steady vapor of fibrin sealant. To prevent the spray-tip from becoming blocked, do not attach to the Pantaject® until immediately prior to use. If the spray-tip becomes blocked, replace it with a new spray-tip.

2. Holding the spray-tip 10-20 cm from the surgical site push down slowly and evenly on the plunger connector to produce a fine, even spray of Beriplast® P Combi-Set. A fine film of fibrin sealant will form on the wound site.

Spray-Set (Berijet®)


Berijet® Spray-Set can be used to apply a fine mist of fibrinogen concentrate and thrombin solutions to large wound areas. The spray-set consists of: 1 spray-head; 1 x 3 m connecting tube with clip and sterile filter. The components of the spray-set are sterilized and are for single use only. The components must not be resterilized or reused.

Berijet® should be used with compressed gas (CO2, air, nitrogen) at a distance of 10-20 cm from the tissue surface and at a maximum pressure of 1.5-2.0 bar.

A clip is attached to the tube to allow the gas supply to be interrupted.

1. Insert the spray-head (1) in the conical recess of the syringe holder.

2. Connect the syringes (2) filled with the fibrinogen solution (blue) and thrombin solution (red) with the spray-head. Snap both syringes into the syringe holder (3). Connect the grip plate (4) to the syringe plungers to prevent jamming of the plungers.

3. Attach the gas tube to the spray-head with the connector (5) and ensure that the clip (6) is closed.

Catheject®

The Catheject® application devices should be used in combination with the 0.5/1 ml or the 3 ml Pantaject® Application Kit or with the CSL Behring Endoscopic Application Kit.

The fibrinogen and thrombin solutions of the fibrin sealant have to be prepared according to the specific package insert coming with the product.

Joint application of fibrinogen and thrombin solutions with the Pantaject® Application Kit


Using sterile technique, remove the Catheject® application device from its pouch. Assemble the application device and the Pantaject® applicator following the appropriate instructions below.

1. Remove the needles from the syringes filled with the fibrinogen solution (blue marking, 3) and thrombin solution (red marking, 4).

2. Insert the Y-piece (1) of the Catheject™ application device in the conical recess of the syringe holder (2), which should be bended at an angle of approx. 45 degrees.

3. Firmly connect both syringes to the Y-piece. The syringe with the fibrinogen solution (blue marking, 3) should be connected to the opening on the Y-piece marked with the F-indicator.

4. Snap both syringes into the syringe holder (2).

5. Connect the grip plate (5) to the syringe plungers by holding the Pantaject® at the syringe plungers. The grip plate prevents jamming of the syringe plungers and ensures smooth forward movement.
